The executive ANCC has in mind

ANCC describes its candidate as a nurse with system-wide or organization-wide influence: usually accountable for several groups of nurses, often for non-nursing staff and operations too, working on strategy, high-level finance, resource management and system integration, and representing the organization outside it (ANCC exam page). The hours rule says the same thing in fewer words: graduate degree + 2,000 h system leadership.

Our practice questions sit at that altitude too: what the executive puts in place so that every unit acts, rather than what one manager does on one unit.

  • NE-BC is ANCC's exam for unit and department leaders, with the same form length and fee grid and a BSN in place of the graduate degree. Which of the two fits your job is settled on NE-BC eligibility.
  • CNML is AONL's exam for nurse managers of a unit, with its pass mark set as a number of correct answers rather than a scaled score.
  • CENP, AONL's executive credential, is not yet covered here; its comparison with this exam is on nurse executive certification.

Four domains, 2025 weights

The Nov 11, 2025 outline spreads the 125 scored questions over four domains:

  • Leadership, 34% (42 questions): strategic planning, governance structures, succession, negotiation, business cases and reimbursement
  • Quality and safety, 26%: high reliability, RCA and FMEA, survey readiness, benchmarking, emergency management
  • Human capital management, 21%: shared and professional governance, healthy work environments, the workforce pipeline
  • Health care delivery, 20%: delivery and payment models, ethics structures, outcome monitoring, AI and technology at system level

Box 1Older splits still circulate

Guides that give Health Care Delivery 27%, Quality and Safety 24% and Human Capital Management 14% describe the 2021 outline. Since Nov 11, 2025, human capital work carries 21% and delivery 20%. Everything else that moved: outline changes.

Question 1 of 5 · Health Care Delivery · 20%

A health system expanding telehealth finds that video visit use is lowest among older, rural, and low-income patients. Which strategy best advances equitable access?

  1. Audio-only visits and help with the technology remove the device, broadband, and skill barriers that keep these groups from video care.
  2. Marketing the portal app online reaches people who are already connected, so it widens the gap for older, rural, and low-income patients.
  3. Prioritizing patients who already use the portal rewards existing access and pushes the excluded groups further behind.
  4. Requiring video for every visit locks out the very patients with the lowest video use, trading equity for a quality claim.
Question 2 of 5 · Human Capital Management · 21%

A hospital's RN workforce is 58% BSN-prepared, below the 80% goal set in the Institute of Medicine's 2010 Future of Nursing report, and many associate-degree RNs cite cost as their barrier to further study. Which program best raises BSN attainment while retaining the nurses supported?

  1. Reimbursement delayed for three years leaves the up-front cost barrier in place, so few nurses can start.
  2. Paying tuition up front removes the cost barrier nurses named, and the later service commitment keeps the newly prepared nurses in the organization.
  3. A one-year mandate is unrealistic for working nurses and pushes experienced associate-degree RNs to leave.
  4. A one-time bonus on finishing arrives after the nurse has already paid, and with no commitment it does nothing to keep the nurse afterward.
Question 3 of 5 · Leadership · 34%

During contract talks, hospital and union negotiators are deadlocked over mandatory overtime: the union demands a total ban and management insists on full discretion. Which approach reflects interest-based (principled) negotiation?

  1. Splitting the difference is compromise between fixed positions, which leaves both sides' real worries about overtime unresolved.
  2. Exploring the interests behind each position (rest and safety for nurses, coverage flexibility for management) opens options neither side's opening stance allowed.
  3. Trading the overtime ban for a smaller raise is classic horse-trading of positions; the concerns behind each demand stay unexamined.
  4. Taking a final offer straight to the staff bypasses the union's negotiators and is a positional pressure move, not interest-based bargaining.
Question 4 of 5 · Quality and Safety · 26%

A monthly run chart of the hospital's fall rate is presented to the board quality committee. Which pattern signals a non-random shift that warrants action?

  1. Comparing a month with last year's average ignores the run-chart rules and turns ordinary variation into a false alarm.
  2. A single record-high month can occur by chance and is not one of the run-chart rules for a non-random signal.
  3. Points alternating across the median are expected random variation, not a signal.
  4. Six or more consecutive points on the same side of the median meet the run-chart rule for a shift, signaling real change rather than noise.
Question 5 of 5 · Quality and Safety · 26%

A Joint Commission survey issues a Requirement for Improvement for inconsistent handoff documentation. What must the organization submit, and when?

  1. A 10-day window does not apply to a Requirement for Improvement; the Evidence of Standards Compliance is due within 60 days.
  2. A root cause analysis held until the next triennial survey confuses a sentinel-event response with the follow-up to an ordinary finding, and the timing is far too late.
  3. A Requirement for Improvement is closed with Evidence of Standards Compliance submitted within 60 days of the final report.
  4. An RCA and action plan within 45 business days is the Joint Commission's sentinel event expectation, not the response to a documentation finding.

Your score shows up after the last answer.

Asked by nurse executives

Do I have to hold the NE-BC before I sit the NEA-BC?

No. NE-BC is not a prerequisite. The two exams match two scopes of role rather than two steps on a ladder, so choose the one that fits the job you hold now.

Is the NEA-BC still a test, or has it become a portfolio?

It is a test. ANCC publishes a current outline and reference list for it, both effective on the same date (Nov 11, 2025).

What share of first-time candidates pass?

73% of first-time candidates in 2025 (706 of 973), per ANCC's yearly certification data. In 2024 it was 70%, in 2023 75%.

Does Magnet require the CNO to be board certified?

No. Magnet sets degree requirements for nurse leaders, not a certification requirement: Magnet requirements.
